Before any training or certification is delivered, all providers must complete credential verification.

Once a training or certification is purchased through the Sundrata Clinic App, providers will be required to:

  • Submit proof of an appropriate professional license, valid in Nevada or applicable jurisdiction
  • Provide proof of current professional malpractice (liability) insurance


All copies of proof of professional license and current professional malpractice (liability) insurance will need to be submitted to contact@sundrataclinic.com, prior to performing at treatment in Sundrata Clinic.

Credential Records & Ongoing Responsibility

Once training or certification is purchased, providers must submit:

  • Proof of an appropriate professional license
  • Proof of current professional malpractice (liability) insurance


Copies of these documents will be securely stored as part of Sundrata Clinic’s credentialing records.

Provider Responsibility

  • It is the provider’s responsibility to ensure that:
    • Their professional license remains active and in good standing
    • Their malpractice insurance remains current at all times
  • Providers must promptly submit updated documentation to Sundrata Clinic whenever:
    • A license is renewed or updated
    • Insurance coverage is renewed, modified, or replaced


Failure to maintain or update required credentials may result in temporary suspension of training, certification, or platform access until documentation is brought up to date.

Credential documentation is retained for record-keeping purposes. Providers are responsible for maintaining current licensure and malpractice insurance and must submit updated documents as required.

Provider Agreement & Assumption of Responsibility

Participation in the Sundrata Certified Provider Platform is subject to acceptance of and ongoing compliance with Sundrata Clinic’s Terms and Conditions, Provider Agreement, and Clinical Policies.

Before participating in any training, certification, or clinical activity, all providers will be required to review and sign a formal agreement confirming that they:

  • Practice as independent contractors, not employees, agents, or representatives of Sundrata Clinic
  • Will comply with all Sundrata Clinic clinical, professional, and operational policies
  • Will practice solely within their licensed scope of practice and applicable laws and regulations

When providing services at Sundrata Clinic:

  • The provider retains sole responsibility for all clinical decisions, professional judgment, treatment planning, and patient outcomes.
  • Sundrata Clinic does not direct, control, or influence the provider’s independent clinical judgment.
  • Sundrata Clinic, its employees, contractors, medical director(s), nursing director(s), founders, officers, and directors do not assume responsibility or liability for the provider’s clinical services, outcomes, or patient care.


All services performed at Sundrata Clinic are rendered by the provider under their own professional license and malpractice insurance.

No Assumption of Clinical Liability of Independent Provider’s Practice by Sundrata Clinic

To the fullest extent permitted by law:

  • Sundrata Clinic shall not be liable for any injury, loss, damage, adverse outcome, complication, or claim arising from or related to:
    • Clinical services performed by an independent provider
    • Acts or omissions of the provider
    • The provider’s failure to comply with applicable laws, regulations, or scope-of-practice requirements


Sundrata Clinic’s role is limited to providing:

  • Training and certification
  • Clinical facilities and equipment access
  • Administrative systems and governance frameworks

Provider Indemnification

As a condition of participation, providers agree to indemnify and hold harmless Sundrata Clinic and its affiliates, employees, medical and nursing leadership, founders, officers, and directors from any claims, liabilities, damages, costs, or expenses arising out of:

  • The provider’s clinical services
  • The provider’s professional conduct
  • Any alleged malpractice, negligence, or scope-of-practice violation


This indemnification applies whether the services are provided on or off the Sundrata Clinic premises.

Providers participating in the Sundrata Certified Provider Platform practice as independent contractors and are solely responsible for their clinical services and patient outcomes. Sundrata Clinic does not practice medicine and does not assume liability for the acts or omissions of independent providers.

How to Book Your Clinical Time

To reserve a time slot in the clinic:

  1. Open the Sundrata Clinic App
  2. Navigate to Scheduling
  3. Select “Independent Provider” as the booking type
  4. Add your name as the Independent Provider delivering the service for that patient
  5. Select the service you will be performing during the appointment
  6. Choose an available date and time
  7. Pay the $300 Platform Access Fee to reserve your clinical time
  8. Enter the patient’s basic information for the scheduled session


A booking is only confirmed once:

  • The Platform Access Fee has been paid, and
  • Patient details and provider attribution have been entered

After Booking: Required Clinical Documentation

Once your booking is confirmed, you must complete all clinical documentation within the Sundrata Clinic EHR system (Symplast Practice).

As part of the booking workflow, you will:

  • Log into Symplast Practice (Sundrata Clinic EHR)
  • Create or update the patient’s chart
  • Schedule the patient’s treatment time in the EHR
  • Send and manage required consent forms

In addition, you will use RxPhoto to:

  • Capture standardized before-and-after images
